Of course it's not just hair, you need to combine facial features, accessories, colors and so on...
When you say 'smooth' do you mean a long, straight and even line (not wobbly), or do you mean that the edge of the lines you draw are jagged/pixellated?
For the first one, if you use SAI, you could set the stabilizer to a higher setting like 15, S-3 etc. It allows you more time for doing the actual stroke so it's easier to make a smooth one.
For the second one it is often an issue with the brush or the file-format you're using, in SAI try setting the quality to 3 or 4.
With firealpaca it is 'Correction' and 'anti-aliasing' settings. I don't really know much about other programs. But I bet they probably have similiar settings somewhere too.
What is Manga Studio like? I've only used SAI and a little photoshop.
